Open 8:30 am - 5:30pm
Monday - Friday.

All other clinics require referrals from our medical clinic.
Visiting the medical clinic first, allows us to refer you to the correct specialist and to have the necessary testing and or procedures done prior to seeing the specialist.

Please be prepared to provide the following patient information when you call:

Name, address and daytime phone number
Insurance
Date of birth
Reason for appointment

What if you don't have insurance? - You may be referred to the Credit and Collection Department to determine if you qualify for financial assistance.
